Why are Hard Stop Violations occurring at Auto-submission when it has not been enabled?

For the past few months, any expense with a violation is being kicked off the auto-submitted report and creating a new report. We have not enabled this Hard Stop Violations feature in our Domain group. It is a huge headache for me nearly doubling the amount of reports I have to handle.

Why is this occurring? How can I fix it?
