CSV report level export has duplicate records. Please advise.

I setup a custom report level csv export template including only report level fields. When reports are exported to the template, all the fields have the correct values. However, there is one row for each expense in the report. For example, if a report with six expenses is exported, there are six duplicate rows in the csv export. Please advise.
